WESTFORD — The Cameron Senior Center is seeking a part-time Elder Outreach Coordinator. The following press release was shared with WestfordCAT and released at the request of the Town of Westford.

The Town of Westford is seeking a qualified part-time Elder Outreach Coordinator (8 hrs/wk) to perform professional, administrative work in developing and implementing the programs and services of the Council on Aging in order to meet the social, emotional, economic and health needs of Westford’s older adult population (55+). 

Working under the direction of the Town Social Worker and Director, duties include conducting office visits with seniors as needed, visiting seniors’ homes for well-being checks and support to isolated seniors as directed, conduct basic assessment of client needs, maintain daily communication log with Outreach Coordinator for continuity of client care, differentiate between basic information and referral services and complex or clinical services, refer clinical and complex cases to Social Worker, provide basic referrals to Elder Services, transportation, private homecare agencies, Elder Law, etc. with client permission.

This position provides outreach and application assistance for SNAP, LIHEAP, administers medical equipment loan program in collaboration with Outreach Coordinator, administers volunteer handyperson program in collaboration with Outreach Coordinator.

Assists Outreach Coordinator and Program Coordinator with planning and administering programs for seniors, in addition to attending trainings, workshops, and conferences as directed.

Must be able understand elder abuse mandated reporting requirements and file reports appropriately, as well as maintain confidentiality of client information and be able to obtain appropriate release of information before sharing client information outside Outreach/Social Services/Director.

Associates Degree preferred in a related field and/or three to five years of experience in the field of gerontology. Current driver’s license, CORI check, First Aid, CPR. Salary is $23.30/hour.
